We are looking forward to getting to know you. Please fill out and sign ALL enclosed forms (6). Plan to bring your current insurance cards, Identification and information to your visit. You will be responsible for your bill if we do not have accurate insurance information. We will file up to 2 insurances for you, if you have a third, we can provide the information for you to file to the third payer. If your insurance requires a referral, please be sure to either bring it with you or check no later than the day before to be sure it was received by our office. If you are not sure if your insurance requires the referral, please contact the number on the back of your card.
There are no returns of any item after it leaves the office, used or unused. Items that may be defective can be returned for exchange.
